2Scheduling Guide
How to Schedule Your Visits
Your screening may require two visits: a Lab Visit (self-scheduled via the location wizard) and a Clinic Visit (arranged by NMS Health using your availability preferences).
Lab Visit
Self-schedule at a participating lab
1On Step 1, enter your address and confirm whether to use that zip for scheduling — this pre-fills the lab search location.
2On the Lab Visit screen, confirm or edit the Search Location field, set a distance radius (start at 10 miles), and click Search Locations.
3Results appear sorted by distance. For each lab, click Hours to check open times, Contact for the phone number, and Services to confirm your required test panel is offered.
4Click Select This Lab on your preferred result. This loads the availability calendar for that location.
5Click Check Lab Availability, then click a green date. A time slot grid appears showing 30-minute windows with open seat counts. Full slots are grayed out.
6Click your preferred time slot — a confirmation banner appears showing the date, time, and lab name. Click Next to lock it in. Or check Walk In to skip booking entirely.
Tip: Bring a valid photo ID and your employer confirmation to your appointment.
Clinic Visit
NMS Health schedules on your behalf
1Appears when your employer's program includes screenings requiring a clinic or pharmacy (e.g., vaccinations, fit tests, vision exams).
2Enter your First Choice — your most preferred day within the employer-approved date window.
3Select a time preference for each date. Choose Available all day for maximum flexibility.
4Repeat for Second and Third Choice — use different days to give the team options.
5After submitting, an NMS Health coordinator will contact you with your confirmed appointment details.
6Need dates outside the window? Email [email protected] before submitting.
Note: Available dates are locked by your employer and cannot be changed within the form.
